Service Users and Unpaid Carers Survey – Experiences during COVID-19

Angus Health and Social Care Partnership would like to hear from people about their experiences of our services during the COVID-19 pandemic.

We would value your input by providing feedback. It is important that we hear about your experiences of using our services.

This will help us to understand what has worked, what didn’t and what improvements could be made. Your experiences and views are important so that we can understand, learn and improve what we do.

We have three surveys that you can choose from. Please select the survey(s) which are most appropriate to you.

All surveys are anonymous and there is no requirement for any personal identifying information to be given on the form.

There is no obligation to complete the survey and this will not affect any services and/or care you receive.
